If your student requires lunch menu modifications due to allergies, please complete the Cafeteria-specific “Special Dietary Needs Form”, MCPS Form 240-23

If students require medication to be administered at school (including over-the-counter medication), please complete “Authorization to Administer Prescribed Medication”, MCPS Form 525-13 (requires physician’s signature).

If epinephrine is required please use “Emergency Care for the Management of a Student with a Diagnosis of Anaphylaxis”, MCPS Form 525-14 (requires physician’s signature). Note that the school requires two epinephrine injectors in their original packaging with the pharmacy label attached. 


Kids After Hours (KAH) requires a separate set of all forms and medications.

About the PTA

What is the Flora M. Singer PTA?

To ensure that the Flora M. Singer Elementary School is the best that it can be, parents, teachers, and other school staff joined together to form the Singer PTA in June 2012 as a non-profit entity of the National PTA.

Singer PTA Goals

  • GIVE all families, teachers and staff the opportunity to develop relationships with others in the community who can answer questions and make everyone feel comfortable in the school.
  • ORGANIZE educational enrichment activities before and after school, including offering scholarships, and fun family activities which are sponsored and/or funded by the Singer PTA.
  • SUPPORT the teachers and staff by providing items needed by the school such as library materials, musical instruments, and showing them appreciation by sponsoring a Staff Breakfast, Staff Appreciation Week and reimbursement for purchasing school supplies.
  • HELP parents navigate the school system. Provide information that families need to support their children in school, such as math and reading workshops. These workshops will be conducted to address the cultural diversity of the school.
  • PROVIDE opportunities to foster a sense of community for all parents, children, and staff by sponsoring community service events such as clothing and food drives!
